Abstract
연구목적: 이 연구의 목적은 치과의원의 정보화를 위한 작업흐름을 형식적인 모델로 구체화하는 것이다. 치과 구성원과 치과 소프트웨어의 역할을 명시하고, 서로 간의 상호작용을 알아보고, 이를 통해서 작업흐름 모델을 디자인하고자 한다.
연구방법: 작업흐름 모델을 디자인하기 위하여, 56명의 치과의사에게 상황적 탐구(contextual inquiry)방법을 이용한 설문조사를 시행하였다. 설문내용은 치과의원의 전산자원에 대한 실태 파악, 치과 구성원 (치과의사, 치과위생사, 데스크직원, 환자)의 역할, 각 치과 구성원과 치과 소프트웨어 사이의 정보의 흐름을 파악하는 것이었다. 그 흐름을 하나의 정형화된 모델로 통합하여 재구성하였다. 치과 구성원의 정보 흐름을 방해하는 것은 무엇인지 파악하였다.
연구결과: 작업흐름 모델을 통해서 살펴본 정보의 흐름은 매우 유기적으로 연결되어 있었다. 각 구성원의 역할과 흐름은 독립적이고 베타적인 것이 아니라 중첩되어 나타났으며, 이러한 결과로 치과 구성원은 복잡한 치과 임상을 비교적 끊김 없이 수행할 수 있었다. 다만, 치료계획을 수립하는 단계에서 치과 소프트웨어 작업흐름이 통합되지 못하였고, 임상적 의사결정을 돕는데 치과 소프트웨어가 활용되지 못하였다. 치료 및 수납을 하는 단계에서 치과 소프트웨어를 활용한 작업흐름은 부분적으로 끊겨있었다. 환자가 직접 치과 소프트웨어에 접속하여 내원 약속을 결정 하는 작업 또한 이루어지지 않았다. 치과 소프트웨어를 통한 작업흐름을 방해하는 가장 큰 이유는 소프트웨어 및 하드웨어의 기능이 때때로 불완전하기 때문으로 꼽혔다.
결론: 치과의원의 정보화는 치과 구성원의 작업을 완벽하게 수행하기 위해 필요한 정보의 흐름이 적절하게 이루어질 때 이루어질 수 있을 것이다. 이는 정보의 흐름을 신뢰할 수 있게 만드는 하드웨어뿐만이 아니라, 환자를 포함한 치과 구성원 사이의 의사소통과 협업이 원활히 이루어지는 치과 소프트웨어를 이용 또는 제작하는 것이 뒷받침되어야 할 것이다.
The purpose of this study was to build a workflow model for paperless dentistry specifying the roles and interactions of the actors in a dental environment.
For designing a dental workflow model, we surveyed 56 dentists using a process called contextual inquiry. The questionnaires and observations included a general overview of individuals, roles, tasks, artifacts and interactions at the dental offices in terms of information flow. Therefore, the model was consolidated and formalized. Interruptions to the workflow were identified.
Dental workflow had highly organized connection involving patients, dentists, dental hygienists, and receptionists. Each of the members with overlapping roles completed clinical work collaboratively and seamlessly. However, dental information was separated with a couple of different dental softwares during treatment planning phase. Computerized clinical decision making was not utilized due to the lack of the software support. Workflow of treatment and fee was partially disconnected. Patients could not make an appointment of their dental visit by themselves. Most of the breakdowns were related to unreliable hardwares and unstable softwares which interrupted dental information flow.
Paperless dentistry could be implemented when there is seamless dental workflow to meet the needs of dental team. It also could be supported with reliable dental hardwares as well as well-designed dental softwares for collaboration between dental team members and communication to/from patients.
